FACILITY/EVENT REQUEST FORM: If church facilities are needed for ministry events (committee meetings, BFG fellowships, etc.) a Facility/Event Request form must be completed and submitted 3 weeks prior to the event. Event reservations can be made on the church website or in the church office, but cannot be made over the phone or by email. The completed form needs to be submitted in the church office for approval in the general staff meeting on Mondays prior to the scheduled date for the church calendar. Even if it is off campus, please fill out the request form. This will help to promote your event.
CHILDCARE/NURSERY REQUEST FORM: FBC Members use these forms when childcare needs to be provided for a scheduled event.
MEDICAL RELEASE FORM: This form is for children who want to participate in any functions with the church. This form is a legally binding document that shows parent/guardian consent of medical decisions in case of emergency and must be notarized. Download & Print.
